Martin BallandClaude Opus 5.5 ce3857c2e5 sign: classify changes by signed object role ...

Judge each object changed after signing by the role it held in the signed
revision, keyed by object number, and compare a slot's accepted generations
across revisions. Adds `Document::accepted_generations` to cos.
